2021- Strengths and Needs Summary 6-20 – Fidelity and High Quality

Wraparound practitioners use this document to gather and record strengths and needs across the 12 Wraparound Life Domains, culture, and vision for both youth and family members. This version aligns with both Wraparound fidelity (National Wraparound Initiative -Phases and Activities, Activity 1.3) and Oregon Best Practices (including CANS items to consider). Additionally it includes a section to reflect any crisis and safety concerns for each Wrap life domain.  This document was implemented in 2021 and modified from the previous version to reduce documentation time and to better meet the needs of the youth, families and practitioners.
