This document is one version of a Wraparound Plan of Care (POC) that includes how to utilize CANS ratings within the planning process. The core elements necessary within POCs are included: Ground Rules; Youth and Family Vision; Team Mission; Prioritized Needs; Prioritized Needs Statements; Outcome Statements; Strategies; Action Steps; and Team Members names and signatures.
