2021- Strengths and Needs Templates – Additional Options to Embed in Plan of Care

This document provides a number of template options for embedding the Strengths and Needs Summary into the Plan of care.  It meets both the Strengths and Needs Summary document requirements and the requirement of the Plan of Care to reflect Strengths and Needs as well.  Imbedding the Strengths and Needs Summary into the Plan of Care combines the 2 documents reducing the number of documents the family receives.
