2018 Strengths & Needs Summary 6-20

Wraparound practitioners use this document to gather and record strengths and needs across the 12 Wraparound Life Domains, culture, and vision for both youth and family members. This version reflects Wraparound fidelity components outlined by the National Wraparound Initiative in the Phases and Activities document- Activity 1.3. It also supports CANS ratings, which are organized by correlating Wraparound life domains.  Reflecting the CANS ratings on the S&N summary is not a requirement, but is a preference for some practitioners and youth and families.
