2021- Wraparound Plan of Care

This is an updated version of the 2020 Plan of Care template that incorporates CANS ratings and domains.  The core elements necessary within POCs are included: Ground Rules; Youth and Family Vision; Team Mission; Prioritized Needs; Prioritized Needs Statements; Outcome Statements; Strategies; Action Steps; and Team Members names and signatures.   It includes the Strengths and Needs in a way that can also act as the S&N summary, if preferred.
