Wraparound Facilitation Knowledge Skills and Abilities Self Assessment Worksheet

This assessment is formatted in the order of the Wraparound facilitation components of a Wraparound meeting. This assessment can be used as a baseline assessment of the Wraparound Care Coordinators knowledge, skills and abilities primarily related to facilitating a wraparound meeting and can also be used to track progress over time. It does includes some questions pertaining to the Wraparound process more generally as well. This assessment is in a yes/no format and provides space to add additional information for each item as well. This assessment can be used as a stand alone assessment or in conjunction with one or both of our other 2 knowledge assessments as well as the wraparound documentation review tool.
