Statewide Coaching: Supporting Successful Transitions in Wraparound

Description:

This coaching session focuses on effective strategies and common challenges related to phase four of the Wraparound process, transition. Coaching sessions are an opportunity for Wraparound providers to share their experiences in supporting families and youth in the transition process, methods for effective transition planning, and what post-transition may look like. Providers are encouraged to send descriptions of challenging situations to trainers ahead of time, or to come with some circumstances in mind to debrief with trainers and colleagues.

Intended Participants:

All Wraparound practitioners (Wraparound care coordinators,care coordinator coaches, care coordinator supervisors, family partners,youth partners, family partner supervisors, and youth partner supervisors) across the state.
